How to Register at the Practice

To register with the practice please complete a Registration Form (GPR) available from Reception.  Where possible we require identification confirmation i.e. Passport or Driving License etc. An appointment for a New Patient Registration Medical should then be arranged either with the Doctor or the Practice Nurse. This is a short medical to enable us to record your medical history and family history and allow us to offer any help and advice we feel would be beneficial to you.  It only takes a few minutes of your time.

Preference of Practitioner

Patients are registered with the practice, not an individual GP.  You can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis. We will do our best to respect your choice. However, not all the doctors in the practice provide all services and specific doctors may not be immediately available.
